Youth Support Request & Consent
Purpose of This Request
This form is used to request and consent to Youth Peer Support & Family Connection services with Bridging Seasons.
This support is non-clinical, grounded in lived experience, and designed to help youth build emotional awareness, communication skills, and self-advocacy — while also supporting healthy family connection.
Participation is voluntary and offered by invitation, after initial conversation and intake.
What This Support Includes
Youth peer support may include:
• Text-based support within agreed-upon boundaries and availability
• Periodic individual support sessions (virtual or in person)
• Optional family or parent check-ins, when appropriate
• Skill-building around communication, emotional regulation, and self-advocacy
The overall goal is to support the youth in developing confidence and skills so they can eventually communicate their needs directly — without needing a third party.
Important Boundaries & Expectations
• This service is not crisis care or emergency support
• Messaging is not monitored 24/7 and responses occur within agreed timeframes
• Youth are encouraged — not pressured — to share at their own pace
• Content of youth communication remains private except when safety concerns or mandated reporting requirements apply
If a youth is experiencing a crisis or immediate safety concern, families should contact 988, local emergency services, or another crisis resource.
Guardian Consent
By consenting, the parent or legal guardian acknowledges:
• They understand the scope and limitations of peer support services
• They consent to their youth participating in text-based and session-based support
• They understand that youth communication is private within professional and legal boundaries
• They agree to respect the process and structure of support
Guardian involvement is collaborative and supportive — not intrusive or supervisory of youth messages.
Youth Acknowledgment (when appropriate)
Youth participation is voluntary.
Youth are encouraged to use this space honestly, respectfully, and safely, and to reach out when support feels helpful.
This support exists to help, not to monitor or control.

Youth Peer Support services are non-clinical and grounded in lived experience.
Bridging Seasons LLC follows Nebraska mandated reporting laws and ethical peer support standards across all services.
Confidentiality is respected within professional and legal limits. If there is concern for safety, harm, or abuse, required reports may be made as outlined in state law.
